NY: Clinic That Treated Joan Rivers Made Mistakes

A report released by the state Department of Health cites Yorkville Endoscopy for numerous deficiencies related to the Rivers case, although negligence is not alleged.

Investigators found conflicting information in Rivers' medical records regarding the amount of the sedation drug Propofol she was administered and the time resuscitation was initiated. They also faulted the clinic for allowing a surgeon who was not a member of the medical staff to perform two nose and throat scoping procedures and noted that a staff member took cellphone photos of Rivers and a surgeon while she was under anesthesia.

Joan Rivers Died From ‘Therapeutic Complications,’ Medical Examiner Says

(CNN) -- Comedian Joan Rivers died of "therapeutic complications" during a procedure to evaluate her "voice changes" and stomach reflux, the New York medical examiner said Thursday.

"The classification of a death as a therapeutic complication means that the death resulted from a predictable complication of medical therapy," the report said.

Rivers, 81, died September 4, a week after an appointment at Manhattan's Yorkville Endoscopy clinic.

The medical examiner's investigation ruled that the cause of death was "anoxic encephalopathy due to hypoxic arrest," a medical description of brain damage caused by a lack of oxygen.

The arrest happened "during laryngoscopy and upper gastrointestinal endoscopy with propofol sedation for evaluation of voice changes and gastroesophageal reflux disease," the medical examiner's officer said.

The complication in this case was laryngospasm -- a spasm of the vocal cords that makes it difficult to breathe, pathologist and surgeon Bill Lloyd told CNN. The situation was compounded when Rivers went into cardiac arrest.

"She stopped breathing and her heart went into arrhythmia because there wasn't sufficient oxygen to maintain the heart muscle," said Lloyd, who was not involved in Rivers' autopsy. "The pump, the heart itself, was unable to move fresh oxygenated blood upstream to the brain."

At first, Lloyd said, Rivers was unable to bring oxygen into her body due to the vocal cord spasm. After a couple of minutes, her heart started to beat irregularly or completely stopped. Finally, her heart stopped delivering fresh oxygenated blood to her brain, which shut down, according to Lloyd.

NBC:  Joan Rivers, Comedy Legend and TV Host, Dies at 81

Joan Rivers, a pioneering female stand-up comic and the queen of "Can We Talk?" gossip, has died. She was 81.

Rivers was undergoing surgery on her vocal cords at a clinic in New York City on Aug. 28 when she stopped breathing and had to be transported to Mount Sinai Hospital. Her daughter, Melissa Rivers, and her 13-year-old grandson, Cooper, who lived with her in Malibu, Calif., rushed to her bedside.


Raspy-voiced and brassy, Rivers was always self-deprecating, foul-mouthed and politically incorrect. A master of reinvention, she endured in show business because of her tenacious work ethic — which she credited to her "immigrant mentality."

Comedians typically push the envelope, but Rivers proved time and again that she didn't even see the envelope. To her fans, she was as shocking as she was endearing.

"The way she is funny, she tells the truth according to herself," the late film critic Roger Ebert wrote in 2010. "She hates some people. She has political opinions. Her observations are so merciless and her timing so precise that even if you like that person, you laugh. She is a sadist of comedy, unafraid to be cruel — even too cruel."

No topic was off limits. From Elizabeth Taylor to Queen Elizabeth to even Anne Frank, Rivers loved going after public figures.

Joan Being Brought Out Of Medically Induced Coma As Family Ponders Suit Over Botched Procedure

Joan Rivers’ daughter is keeping her “fingers crossed” for a miracle to bring the 81-year-old comedic legend back from the brink — even as the family considers legal action over the botched procedure that caused her collapse.

Doctors began lifting the feisty funny lady from her medically induced coma Sunday at Mount Sinai Hospital as Melissa Rivers, her only child, waited to see how the legendary comic would respond.

“The waking-up process has begun and will take until Tuesday,” a source close to the family told the Daily News. “There is real concern that the part of the brain that controls motor skills may have been compromised, leaving her as either a vegetable or in a wheelchair.”

Joan Rivers Rushed To Hospital

8:49 AM PT -- The NYPD tells us ... Joan is currently in critical condition.

We're told ... at one point, Joan's heart stopped beating.

It's unclear if doctors were able to restart the comic's heart.

8:15 AM PT -- We're told an emergency call was placed to 911 at 9:39 AM ET and the caller said, "We have somebody in either cardiac or respiratory arrest."

Joan Rivers was rushed to a hospital in New York City moments ago ... after she stopped breathing during a surgery on her throat ... TMZ has learned.

We're told Rivers was undergoing the throat procedure in a clinic (specifically on her vocal chords) ... when she stopped breathing during the surgery.

Sources say 81-year-old Rivers was rushed to Mount Sinai hospital in New York.

We're told her daughter Melissa is on her way to New York.
